To find out your coverage and cost with your insurance provider, please fill out the contact form on the website. I will send you out my initial intake and consent form. There is a section in the form to input your insurance information. Once you complete the form, I will contact your insurance provider to check your benefits, and I will call you to explain your benefits and get you scheduled for your first appointment.

For Veterans, a referral has to be approved before we can start treatment. You can either submit your referral request through your veteran health portal or have your VA primary care physical submit the referral. The referral will be for acupuncture with Acupuncture in Paradise/ Brennan Vandeveld. NPI is 1699437004. Once the referral is approved and I receive it from the VA, I will contact you to fill out my initial intake and consent form and to get you schedule for your first appointment.
